If you have insurance, your co-payment, and any deductible or co-insurance, is required at the time of your visit. If you are not covered by insurance, you will be required to make a deposit when registering for your visit. Our receptionist can let you know how much you will be required to pay when you register for your visit.
The charges are determined by the services your practitioner provides to meet your unique medical needs. With the wide variety of services available, we can only estimate the charges when scheduling your appointment. The deposit you are required to pay for at your initial visit is based on a low estimate of visit costs. Any charges exceeding the estimated amount will be billed to you.
When making your appointment, you will be asked for your home address, your home and an emergency phone number as well as your insurance type and number. When registering for your appointment, you will be asked to verify your home address, home and emergency contact phone number and a copy of your insurance card will be made. This process may seem like an inconvenience, however, this helps ensure that your insurance company is billed accurately and promptly.
